Swing Bed

Remain in a hospital setting while recovering from illness or surgery and receive continuous nursing care during the rehabilitation period. Additional services that may be utilized would include physical therapy, occupational therapy, speech therapy, and respiratory therapy based on patient need to assist in rebuilding independence to allow them to confidently return to their home or another facility. Discharge planning begins at admission with weekly Care Plan meetings involving the patient's family members and all disciplines involved in the rehabilitation of the patient. Upon discharge, we assist with securing durable medical equipment, home health services, nursing home placement if needed, as well as referrals to agencies for continued medical care and hospice referrals.
