Case Manager
The Nurse Case Manager is a licensed professional member of the Drumright Regional Hospital (DRH) team who provides, directs, and counsels patients/family members to resolve social, emotional, economic or environmental problems which are an impediment to the effective discharge planning that are reasonable and necessary in accordance with the accepted case management practice standards. The position is under the general supervision and direction of Chief Nursing Officer.
DUTIES
1. Demonstrate the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical, and financial outcomes.
2. Consults with patient/family to formulate an individual patient plan that includes needs, goals, expected outcomes, and patient and therapist responsibilities.
3. Treats/counsels patients with appropriate, safe, cost-effective psychosocial care in a timely manner and in accordance with approved standard of therapeutic care, to achieve a positive patient outcome.
4. Provides support/counseling to patients and families by utilizing interpersonal skills, and knowledge to enhance understanding, coping with, and adjustment to illness/condition and treatment/hospitalization.
5. Provides educational and referral information to clients, employers, court systems or other agencies regarding community resources or appropriate treatment facilities by maintaining knowledge of availability.
6. Develops methods to improve critical processes by seeking opportunities and activities for performance improvement, teamwork, and continuing education to enhance service delivery to patients/families, and the entire health care team.
QUALIFICATIONS:
Must be a graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing. A minimum of one (1) year of experience in similar healthcare setting. Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensure in good standing in the state of Oklahoma. Current CPR certification required.
Demonstrate administrative and management skills. Must be able to establish and maintain effective working relationships with the public. Must be able to follow directions and to perform work according to department standards when no directions are given. Must be emotionally mature and be able to function effectively under stress.
The primary focus for this position is Case Management, however, it may require staffing med-surg or the emergency department as needed. Salary varies dependent on licensure and experience.
Additional Information Position Type : Part Time Shift : Day Hourly Wage: $20.00 - $40.00 per/hour
